Due to their poor previous experiences, which include being abused by health researchers, African Americans like Tina Jones are less inclined than other racial/ethnic groups to trust the healthcare system. For this demographic, health literacy hurdles include systemic issues like poor educational prospects, racism, mistrust of the health system, and a lack of culturally appropriate health information and treatments (Muvuka, 2020). Other considerations are cultural belief and attitudes which may influence how an illness is reported or not mentioned in an assessment. For example, mental health issues like depression may not be reported in certain cultures because it may be viewed as being weak. In order to make changes in Tina’s behavior concerning health upkeep with an emphasis on diabetes management and nutrition, mental health, social and cultural influences must be taken into consideration. Knowing she is active in the church may be a way to incorporate educationally backed changes.
In African-American communities, the Black church is a powerful resource for health literacy initiatives. In one promotion, trained church leaders and selected church goers from 21 Black churches to deliver a 6-week church-based health promotion intervention comprising individual coaching, group discussions, and physical activity (Muvaka et al., 2020). At post-test, this initiative significantly increased nutritional health literacy in the intervention group. Similar promotions installed health information touchscreen kiosks in four Black churches to improve health literacy and found self-reported changes in at least one behavior in more than 85% of users 1 to 2 years later (Muvaka et al., 2020). Clergy are usually available as well to help support parishioners with things they may be dealing with. In Tina’s case dealing with her father’s death may help her cope better and she may find support in her church.
Some strategies for cultural competency in Tina’s situation would be, to incorporate well trained minority/ African American into health promotion positions at her church and her college as well as community centers (Betancourt et al. 2005). Having these similar individuals in the community will influence openness in reporting symptoms and expressing feelings which would lead to overall better delivery in improving health conditions and promoting health.
