each subject matter.
To make reports more unified the following template
provides general guidelines for what should be included in each
section.

Individual Adult Patterns of Behavior:
o
This is an overview of their basic functioning
and how they meet their own needs, not a summary of their participation in
services. It should not be more than a
paragraph or two. Please do not cut and
copy your entire court report into this section. This is not a duplicate of the parent summary.

Individual Level Objectives:
o
All ordered or recommended court orders should
be included in an ILO or task.
o
The Family Perspective and Status of Objective
should contain different information, however you can copy over appropriate
sections from your parent summary for efficiency.
o
Old objectives should be removed if they have
been satisfactorily completed.
o
If the Department is recommending something like
Father Engagement, you should add a task to an ILO with a Voluntary status.
o
In the “Additional Services” box in an ILO, only
put recommendations not already in the court order. Do not put N/A or something already captured
in another area. Use a header (i.e. As
recommended in Mr. Smith’s psychological evaluation) because these
recommendations generate into a list on the case plan and can be confusing to
read if you do not state where they come from.

Parent Summary
o
Use headers to clearly present information.
o
Keep previous updates in the court report, with
the newest report at the top.
o
Date the update
o
Safety Threat:
Please start with the current identified safety threat. Please use the safety threat language
followed by what behaviors and patterns supports this threat. Do not use the safety threat numbers, since
they will confuse readers not familiar with our safety framework.
o
Housing
and Employment: Include information about their housing situation and their
employment or how they are supporting themselves. This can include use of state
assistance for financial support.
o
Relationships:
Include information about their relationships.
Are they in one? What pattern do they present with their choice in
partners?
o
The Parent Summary section should also include a
paragraph or sentence about every service the parent has been court
ordered to do and how they are doing in the service. Example of headers for services: Chemical Dependency; Mental Health; Anger
Management; Domestic Violence; Parenting Education.
o
If they are not engaged, remember to add what referrals
and efforts you have made to engage them in each service. Include dates of referrals and provider
information. If a parent has refused to
sign a consent allowing you to make a referral, note this.
o
If they are missing, add a sentence on your
efforts to find them. This can be a
simple ACES check, jail roster check, or info from relatives.
o
Visits:
what is the current schedule? Is the
parent attending regularly? How is their
parenting skills and ability to identify their child’s cues?
o
Compliance
and Progress: Summary of compliance, progress and parental capacity. This
is where you will explain why you are making the compliance and progress recommendations
you have indicated. Note: the CFE doesn’t give the option for “partial
progress,” but the AAG can report
this in court and on the proposed court order.
